ITAC FREE AMPLIFIED PHONE PROGRAM

Just a short time left to qualify for a FREE Amplified Phone, ends December 31, 2007!

The State of Illinois along with ITAC (Illinois Telecommunications Access Corporation) currently provides one free amplified telephone for Illinois residents. In order to qualilfy for an amplified phone, residents must meet the following criteria established by the State of Illinois:

1) Must be a legal resident of Illnois
2) Must have standard phone service in residence.
3) Must have hearing loss documented.

The ITAC applications are available in our office and require a certification by a Doctor of Audiology. The certification states that the applicant has difficulty using standard phone systems and has hearing loss.

To apply for the ITAC program, contact our office at (630) 580-5777 to schedule a hearing test. The hearing tests are billed to the patient's insurance company. We accept Medicare Assignment and most major insurances.
